Substance-free environment: Avoid alcohol and drugs 24 hours before the skydive
Medical conditions: Must declare any of the following by completing a Declaration of Fitness form signed by a doctor
Diabetes, Disease of the brain or nervous system, Dislocated shoulder or other limb(s), Drug or alcohol addiction, Epilepsy, Fits, Heart or lung disease, High blood pressure, Mental illness, Paralysis, Recurrent blackouts or giddiness, Recurrent weakness, Severe head injury, Any past or present medical conditions
